Chanda Spragg
Administrator

Chanda has been with the company for 4 years and has a Bachelor of Science in Health Administration from Fairmont State.  
As the administrator, it is her responsibility to oversee the daily operations of our center.  Her role is very versatile in patient care.  Her main goal is to ensure that each resident has a positive experience during their stay at the center.  Whatever the resident goal is, that will be our goal too, whether it is a short term rehab to home, a transition to long term care or a respite stay.  Throughout a resident's stay, her role as administrator has many facets.  It may be examining a resident' s discharge plans, assisting with a financial problem, working with the nursing department to resolve a clinical issue or helping the family with a concern.

Becky Neff
Business Office Manager

Becky has been with the company for 12 years and has an Associate Degree in Accounting from West Virginia Northern Community College.  
She assists the resident/family if they have questions about or need assistance with billing issues or financial matters including Medicaid, Medicare and private insurance.

Paula Hess
Payroll and Benefits Coordinator

Paula has been with the company for 10 years and is a certified data processing specialist.  
She assists the resident/family with signing money out of the resident's trust fund and with paying bills for the residents who deposit money in a facility account.

Bradley Bowers
Social Services Director

Brad has been with the company for one year.  He has a Bachelor of Arts degree from Marquette University and a Master of Social Work degree and Master of Public Administration degree from West Virginia University.
He is the family and patient advocate at the facility as well as a resident liaison within the community.  He helps to make sure the resident has the best care experience and oversees a smooth transition back home or to the next level of care.

Shannon Hartley
Social Services Assistant

Shannon has been with the company for two years.  
She assists residents with their transition from our center to home.  She coordinates home health agencies and orders any equipment the resident will need in their home.  She works closely with Senior Healthcare in setting up appointments for vision, hearing and podiatry.  She works with the residents and their families to address any concerns they have during their stay at our center.

Tina Rush
Director of Marketing and Admissions

Tina has been with the company for two years.  She has a Bachelor of Science degree in Management from Southern Wesleyan University.  
She is the person to contact when a family is interested in short term rehabilitation or long term placement for their loved one.  She gives tours of the facility and meets with potential resident/family to answer questions prior to placement.  She meets with potential residents in the facility or at area hospitals.  She oversees admission procedures and paperwork, and represents New Martinsville Care & Rehabilitation Center at community events.

Heather Blake
Admissions Coordinator

Heather has been with the company for 13 years.  She is licensed as a Certified Nursing Assistant.  
She is the primary contact for hospitals and individuals who are in need of skilled nursing facility placement.  She guides the transition from home or hospital to our center.  She meets with potential resident/family to do tours and answer questions.  She also completes admission paperwork with them and secures copies of insurance cards and advanced directives.

Anita Holloway
Health Information Manager

Anita has been with the company for 15 years and is licensed as a Certified Nursing Assistant.
She is the caretaker of medical records including filing and thinning of the charts.  She takes pictures of residents upon admission to our facility and inputs the picture and other relevant information into our computer system.

Sherry Hill
Activities Director

Sherry has been with the company for 33 years and is licensed as a Certified Nursing Assistant, an Activities Director and in Reflexology.
She plans all resident activities in the center and coordinates volunteer, student volunteer, and other community service in our center.  She decorates the facility for all holidays.  She works with the interdisciplinary team to devise care plans and goals for all residents, and does progress notes and assessments to monitor their achievement.

Mark Nenadov
Nutritional Services Director

Mark has been with the company for 1 year and holds the designation of Certified Dietary Management.
He ensures that the residents receive delicious and wholesome meals.  He works with our Registered Dietician to meet the residents nutritional needs.

Jeremy Dlugos
Clinical Dietician

Jeremy has been with the company for 3 years and is a registered and licensed dietician.  
He develops plans to fulfill each residents nutritional needs through comprehensive nutritional assessments, weight monitoring, food intake monitoring and nutritional counseling as needed.

Anice West
Director of Environmental Services

Anice has been with the company for 24 years.  
She supervises the laundry and housekeeping staffs.  It is her responsibility to make sure the rooms are clean and sanitized and that the clothes are washed, dried and returned to the resident's room as quickly as possible.  If items are misplaced, she can assist with looking for them and is able to supply clothing for residents if needed.

Alan Jones
Maintenance Director

Alan has been with the company for 3 years and studied maintenance education at Mid Ohio Valley Technical Institute.
He is responsible for the upkeep in the facility and for ensuring equipment runs smoothly for the residents' comfort and safety.  He is eager to assist families with their needs such as hooking up TVs, fixing wheel chairs, etc.

Paula Miller
Director of Nursing

Paula has been with the company for 17 years and is a Registered Nurse.  She has an Associate of Applied Science degree from West Virginia Northern Community College.    
She is in charge of maintaining the nursing staff to properly care for our residents.  She reviews all potential admissions to ensure that we can meet their clinical needs.  She oversees the entire nursing department.  


 


Terry Stillwagoner
Assistant Director of Nursing

Terry has been with the company for 23 years and is a Registered Nurse.  She has an Associate of Applied Science degree from West Virginia Northern Community College and is certified to teach Nursing Assistant classes and holds IV certifications.
She assists the Director of Nursing to ensure the clinical needs of our residents are met.  She oversees the nursing assistants.  She educates the nursing staff as needed on issues such as wounds, negative pressure wound care, g-tubes placements and IVs.  She works with the interdisciplinary team to provide individualized resident care.

Carol Cain
Staff Development Coordinator

Carol has been with the company for 30 years and is a Registered Nurse.  She has an Associate Degree in nursing.  
She is responsible for staff education and development.  She tracks employee health records and oversees infection control in the facility.  


 

Anglia Cross
Clinical Case Manager

Anglia has been with the company for 7 years and is a Registered Nurse.  She has an Associate Degree from West Virginia Northern Community College.
She manages the skilled stay for residents using Medicare or private insurance by helping residents/families determine discharge goals and length of stay at our center.  She assists with other nursing functions and review residents' charts daily.

Gary Burian
Therapy Program Manager

Gary has been with the company for 12 years and is licensed as an Occupational Therapy Assistant.
He is responsible for coordinating therapy services.  He works closely with the Clinical Case Manager to make sure the rehabilitation needs of our residents are being properly met.  He provides direct occupational therapy under the supervision of OTR/L.

Shelby Storm
Speech Language Pathologist

Shelby has been with the company for 4 years and has a Masters degree from the University of Akron.  
She evaluates and treats residents with speech/language deficits, swallowing impairments and cognitive impairment with the goal of helping them return to their prior level of function.

Scott Lucas
Staff Physical Therapist

Scott has been with the company for 4 years and has a Doctorate of Physical Therapy, a Masters of Science degree in Physical Therapy, a Bachelor of Arts in Psychology and Philosophy, and an Associates of Applied Science degree in Occupational Therapy.
He performs physical therapy evaluations to identify resident impairments and develop a treatment plan.  The treatment plan is developed to help the resident achieve their highest level of function and improve their quality of life.  Areas addressed in physical therapy include pain management, wound care, balance/coordination, retraining, contracture management, positioning, wheel chair management, total body strengthening, endurance training and improvement in functional bed mobility, transfers, walking and stairs.

Alyssa May-Pettit
OTR/L


Alyssa has been with the company for less than a year and has a Master's degree from West Virginia University.
She performs occupational therapy evaluations and, when appropriate, develops plans to restore function for the resident to be able to complete self care, home/community management and leisure tasks as independently as possible.  When function cannot be restored, she teaches ways of compensating for the deficit.

Bridgette Crumrine
Physical Therapy Assistant

Bridgette has been with the company for 9 years and has an Associates of Applied Science from Fairmont State, is a certified weight trainer and a senior strength trainer.  
Under the guidance of the physical therapist, she works with the resident to help them regain strength, balance and mobility.  


 


Sarah Bassett
Physical Therapy Assistant

Sarah has been with the company for 3 years and has an Associates of Applied Science from Fairmont State.  
Under the guidance of the physical therapist, she works with the resident to help them regain strength, balance and mobility.

Shainna Broemsen
Receptionist

Shainna has been with the company for 3 years.
Shainna answers the phone, assists and directs residents and their family members in any way she can.  She makes, prints and addressed over 250 newsletters every month.  She keeps track of employment applications, meets and greets visitors, and smiles all the time.
