West Virginia State Police in Tucker County continue to investigate a fatal accident Jan. 16 at Mettiki Coal Mine.

The West Virginia Office of Miners' Health, Safety and Training identified the man as Daniel Lambka, 20, of Kitzmiller, Maryland. Lambka died at 9:10 p.m. Thursday after suffering from crushing injuries, said Leslie Smithson of the Miners' Health, Safety, and Training office.

Lambka had 2 1/2 years of mining experience, 4 1/2 months of which were acquired at the Mettiki Coal's Mountain View Mine. A report issued by the Mine Safety and Health Administration on Jan. 30 said Lambka was struck by a feeder after he had finished connecting a chain between the feeder and a tailpiece. Lambka was standing between the coal rib and the feeder when the securing post dislodged, allowing the tailpiece unit to shift and pin him between the rib and the frame of the feeder.

The Mine Safety and Health Administration is investigating the incident along with State Police.
